A Disaster Preparedness Checklist
(Pet's Survival Kit)

Use this checklist as a guide for building your own disaster preparedness kit for your pet.
(You may want to print it and place it with your pet's file for future reference.)

[ ] Molded plastic pet carrier
[ ] Copy of your pet's rabies and vaccination records
sealed in a plastic bag
[ ] Pet first aid book and kit
[ ] Two-week supply of pet food
[ ] Bottled water
[ ] One month supply of your pet's regular
medications (including flea preventative)
[ ] Non-spill food and water bowls
[ ] Cat litter
[ ] Cat litter pan and scooper
[ ] Leash (and collar with tags attached)
[ ] Plastic bags (for pet waste disposal)
[ ] Sheets (one to cover each pet carrier)
[ ] Blankets
[ ] Non-electric can opener
[ ] Disinfectants
[ ] Paper towels
[ ] Comb/Brush
[ ] Favorite toy or chew
[ ] Recent photo of your pet in case
you are accidentally separated
[ ] Anything else that your pet uniquely needs,
which is not readily available
